Patient Online Access

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • Book or cancel doctor’s or phlebotomy appointments online
  • Renew or order repeat prescriptions online and keep track of your medication

It is also possible to view parts of your GP record. Please select this on the application form.

You can still contact the practice by phone or in person. This is just another option which other patients have found is more convenient and saves them time. Remember, we do not take prescription requests over the phone so Patient Access will help you order your medication.

How to sign-up

  1. Download the Application Form here, fill it out and email it back to us.



2. Next time you visit the surgery, ask at reception to pick up a form.

You do not have to use all the facilities offered. Only tick the box of the online services you wish to have access to.

Online Services for Under 16s

For children aged 16 and under you are able to book online appointments and order repeat prescriptions.

2. Next time you visit the surgery, ask at reception for an application form.

What happens next?

Please allow 7 days for booking appointments and requesting repeat prescriptions and allow 14 days for additional medical record viewing.

Once your application has been processed, the practice will invite you to come in to pick up your login details. It is at this point that you MUST bring in two forms of ID:

  • Photo ID – e.g. passport or driving licence
  • Proof of Address – e.g. utility bill or bank statement

If you are applying for proxy access on behalf of a child under 16, if possible please provide the child’s passport. If you aren’t able to do this then we will need the parent/guardian’s ID (as indicated above) instead.
