• I work for two practices in our Primary Care Network (PCN), primarily The Observatory Medical Practice and secondly Dr Leaver and Partners (Mon, Tue, Thurs, Fri: OMP and Wed: Dr Leavers).
• I enjoy working as part of a multi-disciplinary team in a patient-facing role by taking responsibility for chronic disease management within the practice and undertaking Structured Medication Reviews to pro-actively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple health conditions.
• I provide support with regards to prescription and medication queries, including acute and repeat prescriptions requests and medicines reconciliation on transfer of care for safer prescribing and continuity of care.
• Clinical areas currently working in: Diabetes clinics (area of specialty), Asthma, Hypertension, Ischaemic Heart Disease, Heart Failure and any related medication reviews and queries.
