Changes to registration during Covid-19 Pandemic
Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete an online registration using the links below.
New Patient Registration
To register with Observatory Medical Practive, you will need to fill in a New Adult Patient Registration Form and/or New Child Patient Registration Form. We will need details of your name, address and current GP to assist in the prompt transfer of your medical records.
We request that you provide photo ID and proof of your current address, as well as an immunisation list which can be obtained from your current practice. These can be shown to staff at the reception desk.
For practical reasons we need to ensure that people are who they say they are and check where they live, this will help ensure we match the correct patient details to the NHS central patient registry. Please ensure that you live within the practice boundary.

We will not be able to offer you an appointment to see a doctor or a nurse before you have registered.

Out of Area Registration
If you are out of our area but would still like to register with us, for example because you work locally, please apply in writing via the Out of Area Registration Form.
Non-UK Citizens Registration
Non-UK citizens will need to prove their entitlement to NHS care.
Named Accountable GP
Every patient registered at the practice has a named GP who is responsible for the patient's overall care. However, this does not prevent you from seeing any GP in the practice as you currently do.
At the time of registration you will be / will have been allocated a GP. If you have a preference for who you would like to be registered with, we will make every effort to accommodate you.
If you are already registered with the practice you will already have a named GP. Should you wish to be informed or reminded of your named GP please do not hesitate to contact us. If you wish to change to another GP please let us know and we will make every effort to accommodate your preference.
